We are seeking an Occupational Therapist to join our team at Western CT Home Care Inc. This role is a critical component of our continuum of care at Nuvance Health, where we work closely with patients' physicians and families to provide care in the comfort of their own homes.

The ideal candidate will be a skilled professional who can thrive working independently while being part of a coordinated home health team. Our home health staff enjoy flexible schedules, continuing education options, and excellent medical, dental, and recreational benefits.

Responsibilities
  1. Develops, implements, and evaluates treatment plans in partnership with patients, representatives, caregivers, and physicians.
  2. Provides services ordered by physicians as indicated in the plan of care, communicating with all involved physicians and other healthcare practitioners as needed.
  3. Delivers direct occupational therapy services according to the established plan of care.
  4. Maintains accurate records of client visits, including evaluation, plans for care, client response to therapy, and discharge planning when applicable.
  5. Participates in case management conferences with nurses and other team members to coordinate occupational therapy programs with the total plan of care.
  6. Provides education and counseling to patients and family members regarding occupational therapy treatment regimens.
  7. Identifies patient and family needs for other home health services and makes referrals when necessary.
  8. Takes part in agency quality assurance and performance improvement programs, as well as inservice training.
  9. Supervises care provided by certified occupational therapy assistants and home health aides as required.
  10. Re-evaluates clients' progress and treatment regimes, making changes as needed under physician recommendations.
  11. Fulfills compliance responsibilities related to the position, including inservice training.
  12. Performs additional duties as assigned.

Requirements
Minimum one year of experience; desired experience in qualified home health agencies.

Work Environment
This role involves some exposure to dirt, odors, noise, human waste, etc., significant manual skills/motor coordination/finger dexterity, some occupational risk, and extremely heavy effort.
