Job Summary

We are seeking a compassionate and skilled Nurse (LPN) to join our team at Merrill Gardens at Madison. As a Nurse, you will play a vital role in providing high-quality care to our residents, supporting their physical, emotional, and social needs.

Responsibilities
  • Provide direct care to residents, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans that meet the unique needs and preferences of each resident.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Conduct regular assessments and evaluations to identify changes in resident needs and adjust care plans accordingly.
  • Document resident care and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current LPN/LVN or RN license in the state of Wisconsin.
  • 1-year experience in nursing, preferably in geriatric care.
