About the Role

As a Home Health Occupational Therapist with CenterWell Home Health, you will play a vital role in helping patients regain their independence and live healthier lives. Our team is dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.

Key Responsibilities
  • Confer with patients' physicians and other healthcare team members to develop and revise treatment plans.
  • Review and expand retraining programs to meet the behavioral goals of each patient, within the guidelines of the rehabilitation program.
  • Report patient status and progress to physicians, rehabilitation staff, clinical managers, patients, and family members.
  • Participate in care coordination and discharge planning activities, and serve as a resource to other healthcare personnel in meeting patients' needs.
  • Design community reintegration activities to assist clients in physical reconditioning, psychological adjustment, and coordinate plans with interdisciplinary teams.
  • Recommend and design special adaptive equipment to improve patients' residence or working environments, or enhance their participation in the rehabilitation program and community.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
  • Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluations on the day services are rendered.
Requirements
  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience, with home health experience preferred.
