LPN Home Health and Hospice Nurse

Job Summary:The LPN Home Health and Hospice will provide high-quality patient care in the comfort of patients' homes, focusing on symptom management, education, and support. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities:- Provide patient care according to established standards and policies.
- Assist patients with daily living activities, such as bathing, dressing, and grooming.
- Administer medications and treatments as prescribed by physicians.
- Document patient progress and communicate with healthcare teams.
- Collaborate with patients, families, and caregivers to develop and implement care plans.
- Participate in performance improvement initiatives and quality assurance activities.
- Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
- Minimum two (2) years of Medical-Surgical nursing experience in direct patient care.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
- Ability to create electronic documentation, which is accurate, intelligible, and concise.
