Position Overview:
Fairlane Gardens Nursing and Rehab is currently seeking a dedicated Registered Nurse Assessment Coordinator for our skilled nursing facility.
Role Purpose:
The primary responsibility of this role is to oversee and facilitate the resident assessment process in alignment with state regulations and the facility's policies and objectives.
Key Responsibilities:
1. Conduct regular assessments of residents based on established schedules or as needed due to changes in condition or hospital readmissions.
2. Ensure accurate monitoring, evaluation, and communication of any changes in residents' conditions to the relevant staff.
3. Collaborate with other departments, such as Social Services, Activities, and Dietary, to ensure the completion of assessments and CAAs, securing necessary signatures.
4. Complete MDS sections, quarterlies, CAAs, and care plans in the absence of other staff members.
5. Develop care plans based on identified resident needs in the Resident Assessment Protocols and update them as per guidelines.
6. Prepare and share the MDS schedule with the interdisciplinary team.
7. Submit MDS to the state on a weekly basis (or as required) and maintain precise records, including a transmittal log.
Qualifications:
- Must be a Registered Nurse (RN) licensed in the state of Pennsylvania.
- Previous experience in Long-Term Care as an MDS assessment coordinator is essential.
