Wound Care Specialist

BAYADA Home Health Care is seeking a skilled Wound Care Specialist to provide high-quality care to patients in the comfort of their own homes.
Job Summary:
As a Wound Care Specialist, you will use your clinical expertise to deliver comprehensive wound care services, including wound assessments, treatment plans, and patient education. You will work closely with our interdisciplinary team to ensure seamless care coordination and optimal patient outcomes.
Responsibilities:
- Conduct thorough wound assessments and develop individualized treatment plans
- Provide wound care services, including debridement, dressing changes, and wound cleansing
- Collaborate with healthcare team members to ensure comprehensive care coordination
- Educate patients and families on wound care and management
- Document patient progress and outcomes
Requirements:
- Active and current license to practice as a professional registered nurse (RN) in the state of PA
- Wound, Ostomy and Continence Nurse (WOCN) certification required
- At least one year of nursing experience
- Prior home care experience strongly preferred, but not required
- Ability to travel to cases as assigned
