Job Summary:

We are seeking a skilled and compassionate Hospice Social Worker to join our team at PruittHealth. As a key member of our interdisciplinary team, you will provide essential social work services to patients and their families, ensuring their emotional and psychological well-being during the terminal illness process.

Key Responsibilities:

• Provide professional counseling, assessment, and support to patients and families, addressing their psychosocial needs and concerns.
• Conduct thorough psychosocial histories and assessments, identifying the needs of patients and families, and developing individualized care plans.
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans, ensuring seamless coordination of services.
• Provide crisis intervention and suicide assessment, as needed, and connect patients and families with community resources and support services.
• Document all services and interactions in the medical record, maintaining accurate and up-to-date records.
• Participate in ongoing education and training to enhance skills and knowledge, staying current with best practices in hospice care and social work.

Requirements:

• Masters Degree in Social Work (MSW) from an accredited program.
• Eligibility for state licensure/certification as a social worker, with successful passage of the licensure exam within six months of employment.
• One year of experience in hospice care or a related field, with a strong understanding of social work principles and practices.
• Current drivers license and automobile liability insurance.

Preferred Qualifications:

• Orientation and training to complete assigned duties effectively.
• Ability to work a flexible schedule, with a strong commitment to providing excellent patient care and support.
• Positive criminal history check, Office of the Inspector General Sanction Check, and licensure validations.
• Timely, regular attendance, punctuality, and a professional appearance.
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
