Caregiver/ DSP

Certified Home Nursing Solutions is seeking a dedicated and compassionate Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in assisting clients with daily activities, providing support and care in a safe and healthy environment.

Key Responsibilities:

• Provide direct care and support to clients, promoting their independence and well-being.
• Transport clients to and from our Day Program, ensuring their safe arrival and departure.
• Assist in developing and implementing individualized care plans, tailored to each client's needs.
• Drive an Agency vehicle, adhering to safe driving practices and maintaining a clean driving record.
• Perform physical interventions as necessary, including lifting and transferring clients, while maintaining a safe and healthy environment.
• Establish and maintain positive relationships with clients, fostering a sense of trust and comfort.
• Monitor and maintain a clean, healthy, and safe environment, adhering to company policies and procedures.
• Provide individualized care and support to clients, promoting their independence and participation in activities.
• Communicate effectively with clients, families, and service providers, ensuring seamless care coordination.
• Complete and maintain accurate documentation, adhering to company procedures and regulations.

Requirements:

• High School diploma or equivalent.
• Valid driver's license, with a minimum of 2 years of driving experience and a clean driving record.
• CNA/CMT certification, with a minimum of 1 year of caregiving experience.
• DSP experience, with a minimum of 2 years of experience in a similar role.
