Responsibilities:
- Provide personal care and support services to patients in their homes
- Assist with daily living activities, such as bathing, dressing, and grooming
- Monitor and report any changes in patient condition to the VITAS nurse and/or Team Manager
Requirements:
- One year direct patient care experience in acute or sub-acute care
- Good communication and interpersonal skills
- Ability to work independently and as part of a team
