- Work collaboratively with an interdisciplinary team to provide comprehensive care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide education and support to patients and their families to promote optimal health outcomes.
- Document patient care and progress accurately and thoroughly.
Requirements:
- Current Michigan Registered Nurse (RN) licensure.
- Preferred: Minimum one (1) year of home health nursing experience.
- Graduated from an accredited Wound, Ostomy, and Continence (WOC) nursing education program.
- Experience in wound and ostomy care within an interdisciplinary team.
- Strong interpersonal and communication skills.
- Compassionate patient-focused approach.
- Proper documentation skills and familiarity with medical software.
- Current CPR certification and tuberculosis (TB) test results.
- Ability to pass background checks and drug screening.
- Willingness to travel within designated service areas.
