Job Summary

We are seeking a highly skilled Dietitian to join our team at Advocate Health. As a Cardiac Rehabilitation Dietitian, you will play a critical role in developing and implementing personalized nutrition plans for patients with cardiac conditions.

Key Responsibilities
  • Conduct comprehensive nutritional assessments to identify patients' nutritional needs and develop tailored plans of care.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Provide education and counseling to patients and families on proper nutrition, meal planning, and healthy lifestyle choices.
  • Maintain accurate records of patient interactions, progress, and outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • RD registration with the Academy of Nutrition and Dietetics.
  • Dietitian certification issued by the state in which you practice.
  • Bachelor's Degree in Dietetics or related field.
  • Completion of a CADE-accredited internship or coordinated dietetic program.
  • Intermediate computer skills and excellent communication and interpersonal skills.
Working Conditions

This role requires the ability to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently during the work shift. Occasional lifting of 25 pounds may be required. The successful candidate will also have functional vision, hearing, speech, and touch.
