Job Summary:
As a Care Partner at WellStar Health System, you will play a vital role in promoting a positive patient experience by partnering with the nurse, physician, and healthcare team to provide excellence in patient care. Your primary focus will be to assist with daily living activities and comfort measures, while also performing basic patient care functions such as ADLs, ambulation, and vital signs.

Key Responsibilities:
• Provide resident care according to the written Plan of Care
• Assist Charge Nurse/Manager and other members of the healthcare team with care delivery as assigned
• Provide meals and nourishment to the resident according to dietary standards and the plan of care
• Take vital signs and document, report verbally to the Charge Nurse any unusual findings
• Document all patient care delivered in a timely manner as required by policy

Requirements:
• High School Diploma or equivalent
• Basic Life Support (BLS) certification
• Ability to read, write, and speak the English language
• Basic computer skills required
• Strong customer service and interpersonal skills
• Ability to collaborate with other healthcare providers in the provision of patient care
• Ability to multi-task and function in stressful and emergency situations
