Job Title: Wound Care Physician Assistant

Job Summary:

Theoria Medical is seeking a skilled Wound Care Physician Assistant to join its team to provide high-quality care at skilled nursing facilities (SNFs) across the United States. As a key member of our team, you will be responsible for rounding and managing the wound care of patients residing at our partner facilities.

Key Responsibilities:

  • Rounding and managing wound care for patients at assigned SNFs
  • Collaborating with wound care nursing staff to assess and treat patients with wounds
  • Providing high-quality, patient-centered wound care services to patients residing in SNFs
  • Timely documentation of patient care and encounters
  • Availability for phone and secure messaging consultations with facility nursing staff

Requirements and Qualifications:

  • Valid PA/NP license in the state of practice
  • Active DEA license (Theoria will assist with application for new graduates)
  • General post-acute/SNF knowledge or experience preferred
  • Familiarity with EMR systems and charting practices

Physical Requirements:

  • Ability to move frequently and briskly within facilities
  • Punctual in-person attendance
  • Continuous operation of app-based EMR/notes system
  • Ability to ascend and descend stairs
  • Some ability to position oneself to obtain records and tools
  • Effective communication with patients, managers, and facility staff
