We are seeking a skilled LPN Home Health Nurse to join our team at Bridgewater, MA. As a medical professional, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities:
- Provide high-quality nursing care to patients in their homes, following treatment plans prescribed by physicians.
- Conduct thorough assessments of patients' medical conditions and develop individualized care plans.
- Administer medications, perform wound care, and provide education to patients and their families.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Maintain accurate and complete medical records, adhering to regulatory requirements.

- We offer a competitive hourly rate of $45 per visit or $33-$39 per hour.

Qualifications:
- One year of clinical experience and one year of home health experience.
- Ability to provide nursing visits according to treatment plans and maintain accurate medical records.
Required Certifications and Licensures:
- Graduate of an accredited practical or vocational nursing program.
- Currently licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
- Valid CPR certification.
- Licensed driver with ability to travel to all business locations.
