Job Description

Hospice Chaplains play a vital role in our team, offering spiritual support to patients, family members, and caregivers. Key responsibilities include:

  • Conducting timely spiritual assessments of patients and family members
  • Providing or coordinating spiritual care support
  • Assisting with bereavement follow-up care
  • Participating in care conferences, admissions, and IDG meetings
