Hospice Care Opportunities

PruittHealth Hospice is seeking a compassionate Registered Nurse to join our team as an On Call Weekend RN. As a member of our interdisciplinary care team, you will provide high-quality care to patients and their families, ensuring a smooth transition into our hospice program.

Key Responsibilities:

  • Screen referrals for appropriateness and perform admissions to facilitate the transition of patients and families into the hospice program.
  • Case manage patients and families to ensure symptom management, patient and family education, medication management, coordination of hospice services, and excellent communication with the patient's physician.
  • Coordinate the interdisciplinary care team for assigned patients and implement the plan of care.
  • Provide primary nursing care for hospice patients, regardless of the setting.
  • Assume full responsibility for professional management of assigned hospice patients' care related to the patient's terminal illness.
  • Teach primary caregivers skills needed to care for patients in their residence.
  • Work closely with the attending physician and other team members to develop appropriate pain and symptom management.
  • Complete nursing assessments and admissions of patients.
  • Maintain the clinical record to meet federal, state, and agency regulations and guidelines.
  • Supervise hospice aides and certified nurse aides, making supervisory visits at least every 14 days and face-to-face at least monthly.
  • Assume all on-call responsibilities as assigned.
  • Participate in Performance Improvement Program.
  • Participate in and present inservice programs, team conferences, and staff meetings.
  • Provide skilled nursing care to assigned patients/families as outlined in the plan of care.
  • Participate in the development of the patient/family interdisciplinary care plan.

Requirements:

  • Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
  • Excellent communication skills, both oral and written.
  • Excellent organizational skills.
  • Ability to handle confidential matters.
  • Ability to work a flexible schedule.
  • Commitment to the Hospice concept and standards.
  • Ability to interact with a variety of different age groups.
