Job Summary

We are seeking a highly skilled Care Coordinator LCDC to join our team at the Crisis Center. As a key member of our multidisciplinary treatment team, you will provide substance abuse assessment and counseling services to individuals with a mental illness and co-occurring substance use/abuse diagnosis.

Key Responsibilities:

  • Provide individual, family, and group chemical dependency counseling
  • Develop and implement recovery plans, rehab services, and crisis management interventions
  • Coordinate referrals to inpatient rehabilitation programs and community resources
  • Support sobriety and relapse prevention

Requirements:

  • Bachelor's or advanced degree from an accredited college or university
  • Texas Licensed Chemical Dependency Counselor in good standing
  • Texas driver's license in good standing

Preferred Qualifications:

  • 1 year of work experience providing direct services within a Behavioral Health or Social Service setting
  • Spanish speaking

Work Environment:

  • Must be able to perform SAMA and CPR
  • Must be able to perform moderate lifting and carrying, pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting, and twisting
