Work Hours
FTE hours/week); 11pm-7:30pm
Weekend Rotation: Every Other Weekend
Holiday Rotation: 3 Holidays/Year
Team Collaboration
The LPN in the Inpatient Unit collaborates with a multidisciplinary team, including RN Case Managers, Social Workers, CNAs, and Clinical Assistants.

Key Responsibilities
The LPN plays a crucial role in ensuring patient and family satisfaction by delivering nursing care under the direct supervision of an RN. Responsibilities include:
- Conducting patient visits as assigned, gathering relevant data, and updating care plans.
- Administering medications and maintaining clear communication and documentation.
- Providing patient-centered care aligned with Agrace's philosophy and standards.
- Patient and Family Assessment:
Observe and document the physical, emotional, and educational needs of patients and families as outlined in the plan of care. - Direct Patient Care:
Respond to patient needs, administer medications, assist with daily living activities, and perform nursing procedures as required. - Care Planning:
Understand and follow the plan of care, document all care provided, and oversee the work of CNAs. - Team Participation:
Engage in meetings to evaluate patient services and mentor new staff.
To be considered for this role, candidates should possess:
- Graduation from an accredited Licensed Practical Nurse program.
- Current, unrestricted LPN license in the state of employment.
- Preferred one year of LPN experience.
- Valid driver's license and reliable transportation.
- Ability to maintain professional boundaries and confidentiality.
- Proficiency in Microsoft Office and electronic health records.
