Responsibilities:

  • Provide social work services to patients and families, addressing personal and environmental difficulties that may interfere with optimal health outcomes.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Conduct discharge planning for patients with disabilities or illnesses, assessing needs for specialized care and arranging placements as necessary.

Requirements:

To be considered for this opportunity, you must have a Master's degree in Social Work and be licensed as a Licensed Master Social Worker (LMSW). A minimum of 2 years of experience in a clinical setting is also required.

Compensation:

The estimated annual salary for this position is $85,000-$110,000, based on location and experience.
