We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Giving Home Health Care Careers in Truchas, NM.
Job Summary: As a Home Health Nurse, you will provide patient care in accordance with a physician-established care plan. This role involves administering care at the patient's residence, collaborating with other healthcare professionals, and advocating for the patient's needs.
Responsibilities:
- Provide high-quality patient care in accordance with established care plans
- Collaborate with CNA's, HHA's, Case Managers, and patient physicians to ensure comprehensive care
- Advocate for patients and work one-on-one to meet their care needs
- Work independently with minimal supervision
Requirements:
- Current RN or LPN licensure in the state of New Mexico
- Experience in home health care or a related field
- Ability to pass a background check
