• You must hold a Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree from an accredited dental school.
• You must have completed a post-doctoral residency program for all specialists.
• You must hold a current/active license in good standing in this state (or eligible to obtain).
