The RN Clinical Team Lead is responsible for ensuring that patient care is coordinated and managed effectively. This role requires strong leadership and communication skills to supervise clinical personnel and ensure that care and services are delivered appropriately.
Key Responsibilities- Ensure an RN Clinical Manager/Supervisor is available during all operating hours.
- Develop, implement, and update individualized plans of care in collaboration with physicians and other healthcare professionals.
- Review patient information and assign home care personnel as needed.
- Confer with Attending Physicians regarding patient eligibility for services.
- Review and evaluate each case by reviewing services provided, conferences, record review, and discussing impressions with clinicians.
- Assist clinicians in establishing therapeutic goals, setting priorities, and developing plans of care.
- Attend and run case conference meetings to facilitate coordination of care.
- Participate in quarterly record reviews and communicate findings to the Director of Nursing.
- Assist in the screening and interviewing process of new personnel and make recommendations for employment.
- Assist in the orientation of new personnel.
- Comply with accepted professional standards and principles.
- Must be a registered nurse with current licensure to practice in the State.
- Previous experience in home care setting required, with two years management or supervisory experience preferred.
- Complies with accepted professional standards and practice.
- Have excellent observation, good nursing judgment, and communication skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Complies and maintains current CPR certification.
