Position Overview:
As a Registered Nurse at Chapters Health System, you will play a vital role in delivering exceptional care to patients and their families within the Hospice House setting. This position requires a commitment to excellence in patient service and a passion for making a difference in the lives of those facing terminal illnesses.

Key Responsibilities:
1. **Patient Assessment:** Conduct thorough evaluations to identify the needs of patients and their families, utilizing the nursing process effectively.
2. **Care Coordination:** Collaborate with the Interdisciplinary Group (IDG) to develop and implement individualized Plans of Care that reflect the evolving needs of patients.
3. **Education and Support:** Provide education to patients, families, and caregivers regarding disease processes, palliative care interventions, and safety practices.
4. **Communication:** Maintain clear and accurate communication with all members of the healthcare team, ensuring that any changes in patient conditions are reported promptly.
5. **Supervision:** Oversee the care provided by nursing team members, including LPNs and Hospice Aides, ensuring high standards of care are met.

Qualifications:
- Current Florida RN license or valid multistate/compact license.
- Minimum of one year of nursing experience in an inpatient or hospice setting.
- Proficiency in using Electronic Medical Records (EMR) systems.
- Strong verbal and written communication skills in English.
- Ability to manage the emotional challenges associated with caring for terminally ill patients.

Physical Requirements:
This role requires the ability to perform essential physical tasks, including lifting, bending, and standing for extended periods. Flexibility in scheduling is necessary to meet the needs of patients in the Hospice House.
