Key Responsibilities
  • Provide 1:1 patient care to pediatric patients in their homes, ensuring their physical and emotional needs are met.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments, identify areas of concern, and develop strategies to address them.
  • Administer medications, perform treatments, and provide education to patients and families.
  • Document patient progress, maintain accurate records, and participate in quality improvement initiatives.
Requirements
  • RN/LPN with a valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills.
  • Ability to maintain confidentiality and handle sensitive information.
