The LPN MDS Coordinator plays a vital role in the Resident Assessment Process by completing relevant items on the Minimum Data Set (MDS) and the associated Care Area Assessments (CAAs) to ensure optimal patient care and appropriate nursing center reimbursement.
Key Responsibilities:
- Oversee the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for completion, computer encoding, and electronic State transmission of the MDS.
- Contribute to the development of personalized resident Care Plans, identifying resident needs and expected outcomes.
- Educate new interdisciplinary team members on clinical content and the electronic completion of applicable MDS items.

Note: Restrictions may apply based on collective bargaining agreements, state laws, job classification (full-time, part-time, or casual), job grade, location, and length of service.
Qualifications:
- Previous experience in a hospital, long-term care facility, or similar healthcare environment is required.
- Knowledge of general, rehabilitative, and restorative nursing practices, as well as laws and regulations governing long-term care, is essential.
- Proficiency in computer skills, including Windows applications, is necessary.
- Prior experience in completing MDSs and Care Plans is required.
