Job Title: Wellness Nurse

We are seeking a compassionate and skilled Wellness Nurse to join our team at Eden Senior Care. As a Wellness Nurse, you will play a vital role in providing high-quality patient care and promoting a positive living environment for our residents.

Job Summary:

The Wellness Nurse will be responsible for planning, implementing, and evaluating patient care plans to ensure the best possible outcomes for our residents. This includes assessing patient needs, developing and implementing care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care delivery.
  • Conduct regular assessments to identify changes in patient condition and update care plans accordingly.
  • Supervise and train nursing assistants to ensure they are providing high-quality care.
Requirements:
  • Current LPN or RN license in good standing.
  • Minimum 1 year of experience in long-term care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Compassionate and empathetic approach to patient care.
