Hospice Liaison

Amedisys, a leading home health and hospice company, seeks a skilled Hospice Liaison to join their team. The ideal candidate will have experience in outside sales and business development within the healthcare industry, with a background in cold calling and customer engagement.

• Develop and maintain relationships with key referral sources, including hospitals, nursing homes, and physician offices.
• Coordinate patient admissions and obtain required documentation to complete the referral process.
• Conduct on-site visits to assess patient needs and identify barriers to care.
• Keep accurate records of patient interactions and referrals.
• Achieve admissions, product, and payor-mix goals.

• At least one year of experience in a healthcare-related business development or outreach role.
• Reliable transportation, current driver's license, and liability insurance.

• A competitive salary range of $75,000 - $80,000 (based on experience).
