Job Summary:
VNS Health is seeking a skilled Nurse Manager to lead our Care Coordination team. As a key member of our organization, you will be responsible for managing the day-to-day operations of our care coordination services, ensuring that our members receive high-quality, comprehensive care.
Key Responsibilities:
- Lead a team of care coordinators to achieve excellence in care coordination and member outcomes
- Develop and implement process improvement plans to enhance the quality of care and services
- Collaborate with multiple departments to achieve quality care, compliance, and member outcomes
- Use software systems, including Excel dashboards, to manage processes, data, and performance metrics
- Coach team members on productivity and performance, fostering collaboration and decision-making
- Maintain a varying compliance environment to ensure adherence to policies and guidelines
Requirements:
- License and current registration to practice as a Registered Professional Nurse in the state of New York
- Bachelor's Degree in nursing or Master's Degree in Social Work
- Minimum five years of experience in home care, care management, or related field
- Minimum of two years progressive experience in clinical management
- Demonstrated knowledge of NYS regulations regarding Medicare or Medicaid Managed Long Term Care health plans
