Responsibilities- Implement and Monitor Patient Care Plans
- Develop and implement individualized patient care plans in collaboration with the healthcare team.
- Monitor patient progress and adjust care plans as needed.
- Coordinate Patient Care
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions.
- Assess and Coordinate Patient Discharge Planning
- Assess patients' discharge needs and develop plans to ensure a smooth transition to post-acute care.
- Coordinate with healthcare team members to ensure all necessary arrangements are made for patient discharge.
- Education and Experience
- Graduate from a fully accredited RN program.
- At least 1 year of ICU/Critical Care experience required.
- Certifications and Licenses
- Active unencumbered RN license in the state of employment.
- BLS and ACLS certifications required.
- Skills and Abilities
- Strong technical skills and ability to work with electronic medical records.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
