Job SummaryWe are seeking a skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Hospice team as a Case Manager. As a Case Manager, you will be responsible for providing comprehensive care to our patients, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities- Assessment and Planning: Conduct ongoing assessments of patients' physical and psycho-social status, and develop plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and their families.
- Pain and Symptom Management: Provide education and interventions to support effective management of patients' pain and other physical symptoms, and assess patients' and families' responses to interventions and treatment.
- Equipment and Supplies: Ensure that necessary equipment, supplies, drugs, and biologicals are available to support patient care.
- Documentation and Communication: Complete accurate and timely documentation of patient services provided, and regularly attend IDG meetings to ensure seamless communication and collaboration.
- Licensure and Certification: Active, unrestricted RN or LPN license in the State of Michigan, and current CPR certification.
- Experience: At least one year of relevant experience as an RN or LPN, with preference given to those with management experience, home health experience, and community health experience.
- Transportation: Reliable transportation with a valid state driver's license and current car insurance.
- Weekly Pay: Competitive weekly pay.
- Mileage Reimbursement: 54 cents per mile.
- Tablet and Wi-Fi: Tablet with built-in Wi-Fi provided.
- Paid Training: Comprehensive paid training.
