Job Summary

York Hospital is seeking a highly skilled and compassionate Registered Nurse to join our Family Practice team in Kittery, Maine. As a Family Practice Nurse, you will be responsible for delivering high-quality patient care, assessing, coordinating, and participating in the total care and teaching for patients. You will work collaboratively with patients, family members, physicians, peers, and other healthcare team members to create exceptional patient experiences.

Key Responsibilities
  • Assess and coordinate patient care, including developing and implementing individualized care plans
  • Provide education and support to patients and their families on health-related topics and disease management
  • Collaborate with healthcare team members to ensure seamless care transitions and optimal patient outcomes
  • Participate in quality improvement initiatives to enhance patient care and safety
  • Maintain accurate and up-to-date patient records and documentation
Requirements
  • Current RN license in the state of Maine
  • Minimum 2 years of experience in Family Practice or Triage nursing
  • BCLS certification
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
