Key Responsibilities
  • Effectively screen and evaluate patients with cognitive and physical functioning disorders, developing individualized plans of care that meet regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practices to support clinical interventions, providing rehabilitative, skilled, and medically necessary treatment to patients.
  • Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and modalities.
  • Assess patient progress and adjust treatments accordingly, evaluating treatment outcomes for effectiveness and making referrals as needed.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
  • Effectively communicate with supervisors and other healthcare team members regarding patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries of services provided, meeting regulatory and clinical requirements.
Qualifications
