About the Role

We are seeking a skilled LPN Team Case Assistant to join our Home Care team at Rochester Regional Health. As a key member of our team, you will play a vital role in coordinating patient services and ensuring the highest level of care for our patients.

Key Responsibilities
  • Coordinate patient schedules and prioritize daily tasks to ensure efficient care delivery
  • Request additional services such as aide, therapy, and social work as needed
  • Ensure timely referrals for services and coordinate with team members to achieve optimal patient outcomes
  • Provide exceptional customer service and respond to patient inquiries in a timely and professional manner
  • Support field staff with day-to-day issues and provide resources as needed
  • Collect, organize, and review program data for reporting and follow-up purposes
  • Perform clerical duties, including answering phones, documentation, and ordering supplies
Requirements
  • Graduate of an accredited school of licensed practical nursing
  • Valid NYS license as a Licensed Practical Nurse
  • Proficient use of Microsoft Office
  • One year of experience in a home care setting or medical office preferred
