Clinical Care Specialist

The Mass General Brigham team is seeking a skilled Clinical Care Specialist to provide high-quality, patient-centered care in various settings. This role offers the opportunity to work with a diverse patient population and contribute to the delivery of exceptional healthcare services.
About the Job:
This position involves providing clinical services to patients in various settings, including home care and community-based environments. The ideal candidate will possess strong communication and interpersonal skills, as well as the ability to work effectively within an interdisciplinary team. A minimum of one year of previous nursing experience in an acute care setting is required, with prior home care experience preferred. Travel requirements may apply within the MGB Healthcare at Home geographic area.
Key Responsibilities:
- Deliver high-quality, patient-centered care in various settings.
- Collaborate with interdisciplinary teams to develop and implement effective care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Qualifications and Experience:
To be successful in this role, candidates should possess:
- A graduate degree from an accredited School of Nursing.
- A current Massachusetts license as a Registered Nurse.
- A minimum of one year of previous nursing experience in an acute care setting.
- Prior home care experience is highly preferred.
