Key Responsibilities:
  • Evaluate patients to determine their physical therapy needs and develop personalized treatment plans.
  • Provide one-on-one physical therapy sessions to patients in their homes, utilizing various techniques and modalities to promote optimal health outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless coordination of care.
  • Participate in multidisciplinary care conferences to discuss patient progress and develop comprehensive care plans.
  • Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and discharge summaries.

Requirements:
  • Graduate of a Physical Therapy Program approved by the Texas State Board of Physical Therapy Examiners.
  • Licensed by the Texas State Board of Physical Therapy Examiners.
  • BCLS certification required.
  • Over one year of combined experience in acute orthopedic, neurology, skilled nursing, out-patients, home health, and wound care preferred.

Salary:
$85,000 - $110,000 per year, depending on experience.
