Nurse Practitioner

The Licensed Practical Nurse is a vital member of our healthcare team at Pasco Southwest Home Health. As a skilled and compassionate caregiver, you will provide direct patient care under the supervision of a registered nurse, ensuring the highest level of quality and safety for our patients.
Key Responsibilities- Implement the plan of care initiated by the registered nurse, providing accurate and timely documentation consistent with the plan of care.
- Collect assessment data and perform observations to report to and collaborate with RN(s) and physician(s).
- Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
- Use equipment and supplies effectively and efficiently.
- Participate in personal and professional growth and development.
- Perform other duties as assigned by the registered nurse.
- Must have completed a requisite practical (vocational) nursing program or be licensed in the state where the LPN is located.
- Has three years nursing experience preferred. Community/home health or medical/surgical experience is preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possesses and maintains current CPR certification.
