KEY RESPONSIBILITIES:
The Lead Case Manager is a vital role responsible for overseeing case management and ensuring safe and timely service delivery, mentoring new case managers, and supervising the activities of the case management team. This position guarantees effective oversight and coordination of case management services.
Supervisory Duties:
- Distributes caseloads of children requiring intensive case management and essential services, including medical evaluations, educational assessments, legal support, and family reunification efforts.
- Guides, supports, and trains Case Managers and the Sub-Lead Case Manager.
Core Responsibilities:
- Ensures thorough completion of assessments and any associated social or medical history documentation relevant to the client's needs and case records.
- Facilitates the service planning process; engages in client staffing meetings and follow-ups as assigned.
- Guarantees the accurate and timely completion of all required documentation, including administrative, clinical, or caseload obligations.
- Safeguards the human and legal rights of clients.
- Delivers direct services to children to maintain appropriate staff/client ratios in compliance with regulatory standards.
- Provides high-quality services while achieving productivity targets as established by the supervisor.
- Adheres to all policies and procedures as mandated by funding sources, state licensing requirements, and agency guidelines.
- Responds to crises and offers intervention support.
- Applies for necessary support funds as required.
- Performs additional duties as assigned.

QUALIFICATIONS:
- Bachelor's degree with a minimum of three (3) years of progressive experience demonstrating supervisory and case management skills, along with two (2) years of experience working with children. A Master's degree in behavioral sciences, human services, or social services is preferred.
- Commitment to at least 10 hours of continuing education/training annually.
- Bilingual proficiency in English and Spanish is required.
- Proof of immunizations for Varicella (2 doses), MMR (1 dose), Influenza (annual), and Tdap (1 dose).
- A valid driver's license with auto insurance coverage is necessary.
