RN Case Manager

Central City Concern is seeking a highly skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will work closely with our multidisciplinary team to provide intensive case management and integrated dual diagnosis treatment to individuals experiencing homelessness.
Key Responsibilities:- Conduct comprehensive assessments and develop individualized treatment plans
- Provide direct care and delegate care to patients as indicated by patient needs
- Triage and assess clients in clinic and community settings
- Participate in clinical staff meetings and case conferences with team members
- Formulate individualized nursing care plans and participate in patient care meetings
- Administer medications and treatments as directed by standing orders
- Consult with other health team members to lend guidance as necessary
- Collaborate with residential facilities, adult foster homes, or other housing providers to provide necessary treatment services and coordinate residential and non-residential treatment and service planning
- Participate in admission and discharge planning for any client needing hospitalization or subacute level of care
- Monitor and assure that the health and safety needs of clients are met
- Coordinate services with all members of client's support network
- Maintain client records as required by program standards and funding requirements
- Participate in the rotation of carrying after-hours pager and providing holiday coverage with other members of team/program
- Attend all mandatory CCC meetings and trainings in a timely manner
- Successful completion of a course of accredited study that leads to the AD, or BSN or AA degree in Nursing from OSBN
- Current RN license in the State of Oregon
- Current certification with MHACBO as a QMHA-R or QMHA I
- A minimum of two (2) years experience in psychiatric or addictions nursing
- Must possess a current driver's license
- Must be available for after-hours pager and holiday coverage rotation
- QMHP
