The Clinical Care Specialist will play a vital role in delivering high-quality patient care at Altru Health. This position involves performing and documenting assessments, ensuring health history and admission processes are completed, and identifying discharge planning and education needs. As a key member of the multidisciplinary team, you will contribute to the implementation of an integrated patient-centered care plan.

Responsibilities include assisting registered nurses or other licensed practitioners in identifying patient needs, priorities of care, and goals based on patient age, ethnic origin, economic status, lifestyle, creed, or philosophical beliefs. You will also gather, observe, record, and communicate patient responses to nursing interventions, modifying the plan of care in collaboration with licensed practitioners as needed.

To succeed in this role, you must demonstrate competency and ability to carry out department-specific functions in accordance with the department's standards of care, policies, and procedures. Other duties may be assigned to meet the needs of the department/organization.

We require a Licensed Practical Nurse (LPN) with a strong focus on providing quality patient care. A minimum of a current LPN license is necessary, along with Basic Life Support (BLS) certification from the American Heart Association within 90 days of start date. Temporary LPN licenses are accepted prior to start, but an unencumbered nursing license is required by start date.
