Public Health Nurse

We are seeking a skilled Public Health Nurse to join our team at the Texas Department of Aging & Disability Services. As a Public Health Nurse, you will play a critical role in providing routine public health nursing work in clinical and non-clinical settings for the assessment, care, and treatment of individuals, families, and communities.
Key Responsibilities:- Provide public health nursing services in the clinic, home, and community, utilizing components of the nursing process to assess, plan, implement, and evaluate the physical, emotional, and psychosocial health status of clients, families, and communities.
- Provide individualized care according to policies, procedures, program standards, physician orders, and standing delegation orders, which may include lab testing/counseling, independent home visits, providing/administering medications, education, and counseling, case management, referrals, and follow-up.
- Participate in community assessments, identifies health needs, plans, and implements strategies, and evaluates outcomes in conjunction with community stakeholders/partners and Regional work plans.
- Develop and implement community health education programs, establishes and maintains communication and partnerships with stakeholders, including local/county officials, community leaders, other agencies, groups, healthcare providers, schools, and the general public.
- Knowledge of nursing techniques and procedures, public health laws and regulations, community health and nursing care principles, practices, and procedures, including culturally appropriate care, and quality assurance principles and procedures.
- Ability to plan, coordinate, and evaluate the delivery of direct public health services, conduct community assessments, provide patient care, prepare and maintain records, and communicate effectively via written and verbal communication.
This position requires travel and may participate in disaster response and/or recovery or Continuity of Operations (COOP) activation. The employee actively participates and/or serves in a supporting role to meet the agency's obligations.
Education and Experience:A Bachelor's degree in Nursing or a related field and a current RN license in the state of Texas are required. Experience in public health nursing and community health education is preferred.
