LHC Group is seeking a Physical Therapist to provide high-quality care to patients in Emerson, New Jersey.

Job Summary

We are looking for a skilled Physical Therapist to join our team in Senior Living. As a Physical Therapist, you will have the opportunity to build trusted relationships with patients, provide continuing education and tuition reimbursement opportunities, and enjoy independence and autonomy in your work.

Responsibilities
  • Evaluate patients to determine rehab potential and establish individualized treatment plans.
  • Collaborate with patients and families to set goals and achieve them.
  • Document each visit in the EMR and complete D/C summary on each patient's final visit.
  • Participate in Department Team meetings and conduct in-services for other staff members as needed.
Requirements
  • Graduate of an accredited school of Physical Therapy and successful completion of the National Registry exam.
  • Current Physical Therapy License in the state of practice.
  • CPR certification and all other certificates and documents as required by LHC Group policy.
