Nurse Practitioner

Nuvance Health is seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will provide direct patient care that is guided by the American Nurses Association (ANA) scope of practice.
Key Responsibilities- Assess patients for potential safety risks and adhere to hospital policies and procedures regarding patient safety.
- Collaborate with patients, family/significant others, peers, physicians, and other members of the interdisciplinary healthcare team in the delivery of healthcare services to patients.
- Develop and implement individualized care plans that address the physical, emotional, and social needs of patients.
- Document detailed information incorporating the plan of care, interventions, and outcomes reflecting the patient's physical, mental, and emotional status.
- Ensure appropriate documentation based on the level of care the patient is receiving to include transfer/discharge documentation as indicated.
- Follow the five Rights of Medication Administration and verbalize to patients/family/significant other the medication they are receiving.
- Assess the patient prior to administering medications noting allergies, parameters, potential side effects, effectiveness, and intervening as needed.
- Reassess and document response according to hospital policy.
- Coordinate and supervise the delivery of nursing care, including the delegation of tasks.
- Maintain accountability for care delegated.
- Actively work to promote a culture of safety.
- Conduct bedside shift report and bedside rounds with peer on assigned patients at change of shift.
- Demonstrate quality by documenting the application of the nursing process in a responsible, accountable, and ethical manner.
- Ensure patient hand-off as appropriate when leaving for breaks.
- Assess learning needs of patient/family/significant other and plan and provide appropriate instruction using applicable teaching methods.
- Assess and document patient/family/significant other response to teaching.
- Care for specific patient populations, despite location, as necessary.
- Deliver patient-centered care by embracing the cultural diversity and difference of patients and team members.
- Respect the dignity of all individuals through the maintenance of the caring relationship.
- Maintain open lines of communication inclusive of actively listening.
- Meet accreditation, regulatory, and compliance responsibilities related to the position.
- Work to promote a culture of learning and growth by seeking experiences and formal and independent learning activities to maintain and develop clinical and professional skills and knowledge.
- Teach others to succeed through mentorship.
- Perform other duties as assigned within the state nursing scope of practice.
- Graduated from an accredited school of nursing with an Associate's Level Degree (required) or Bachelor's Degree in Nursing (preferred).
- Current and unobstructed New York State Registered Nurse (NY Hospitals) license.
- Current and unobstructed Connecticut Registered Nurse License (CT Hospitals).
- New graduates awaiting licensure at the discretion of the Chief Nursing Officer (CT only).
- American Nurses Association endorsed Certifications or Academy of Medical-Surgical Nurses Certification Preferred (*Based on Certification specialty hours of work requirement).
This position requires significant manual skills/motor coordination and finger dexterity. The occupational risk is significant, and the physical effort is very heavy, exerting up to 50 lbs. force. The physical environment is significant exposure to dirt, odors, noise, human waste, etc.
