Job Summary:

The Licensed Practical Nurse will administer nursing care to designated patients under the direction and supervision of a registered nurse. This includes performing direct and indirect patient care duties, effectively interacting with patients, healthcare members, and other agencies, and maintaining standards of professional nursing.

Key Responsibilities:
  • Perform direct personal resident care to an assigned group of residents, following the resident Comprehensive Care Plan and care profile.
  • Administer medications according to facility policy and general principles of safe medication administration.
  • Complete treatments and procedures according to facility policy.
  • Direct CNA's in the performance of their job duties.
  • Observe, evaluate and report significant resident signs/symptoms and/or changes in health care status to the supervising RN or physician, as necessary.
  • Document resident care/problems and status in compliance with facility policy and mandated regulations.
  • Complete all required documentation in consistent, accurate and timely manner.
  • Demonstrate correct and safe technique in the use of equipment such as beds, side rails, scales, Hoyer lifts, bedchecks, whirlpool tub, safety devices, intercom, call bell system, gait belts, oxygen and suction equipment, tube feeding equipment, oximetry machine, BG machine.
  • Maintain nursing equipment and nursing unit in a clean, safe and orderly manner.
  • Participate in the data collection process for the Q.A. program as directed.
  • Complete resident/family teaching as directed by the RN.
  • Coordinate all aspect of unit operations when in Charge position including, but not limited to: assignment of resident care loads to appropriate team member, assignment of staff breaks/meal times, rounds on residents, and monitor assigned staff for completion of their assigned duties.
  • Address and report to Nurse Manager/Supervisor any problems identified with staff and/or residents.
  • Complete 24-hour report and shift to shift report to ensure follow-through on all issues of concern related to the status of each resident
  • Provide input as requested into employee evaluations.
  • Complete/monitor preventative skin care measures for residents.
  • Read, sign and follow all memos related to nursing department and unit function.
  • Maintain confidentiality of all resident information.
  • Participate in rounds with unit physicians as requested.
  • Prepare information for MD/Nurse Practitioner rounds as requested.
  • Transcribe, double-check and care out MD/Nurse Practitioner orders.
  • Assist in the orientation of new employees as requested.
  • Assist in the completion of admission and discharge paperwork as directed.
  • Assist in providing for the physical, emotional, and spiritual comfort and environmental safety of residents.
  • Perform other duties as assigned.
Requirements:
  • Graduate of an approved LPN program required.
  • Current NYS licensure as Licensed Practical Nurse required.
  • Current certification in CPR preferred.
  • Completion of Psychopharmacology and Emergency IV courses preferred.
  • 6 Months to 1 year of experience in similar hospital setting preferred.
Working Conditions:

The Licensed Practical Nurse will work in a fast-paced environment and will be required to frequently walk, stand, bend, lift, push and pull during the work day. The position requires pushing beds, wheelchairs and recliners in excess of approximately 200 pounds with assistance. The Licensed Practical Nurse will also be required to lift, carry, reposition, and transfer residents who may weigh in excess of 200 pounds using proper body mechanics.

The position requires unit specific orientation and attendance of various meetings and participation on committees as required. The Licensed Practical Nurse will also be required to actively participate in in-service programs (12 hours annually), in Quality Assurance programs and CQI, cross training; attends all mandatory classes as required; completes unit-specific competencies.

The position requires assignments to other units/shifts as hospital needs require. The Licensed Practical Nurse will also be required to comply with DOH requirements for annual health assessments and required immunizations.
