Concordia Visiting Nurses is a Medicare-certified, state-licensed, and Medicaid-approved home health agency serving nine Western Pennsylvania counties. We are seeking a skilled and compassionate Nurse Practitioner to join our team in the South Hills service area.
Job Summary:The Certified Registered Nurse Practitioner (CRNP) will collaborate with our Clinical Team to implement and evaluate patient plans of care, ensuring the highest quality of healthcare is delivered. This full-time position involves providing skilled nursing care to patients in their homes, facilities, or hospitals.
Responsibilities:- Provides skilled nursing care, including teaching, treatments, and health promotion practices.
- Executes preventive, rehabilitative, and restorative nursing procedures to ensure patient safety and well-being.
- Assesses patient signs and symptoms, reports reactions to treatments, and makes revisions to the plan of care as needed.
- Develops and implements patient care plans in collaboration with physicians, patients, and designated representatives.
- Supervises and evaluates the care provided by Licensed Practical Nurses (LPNs) and home health aides.
- Coordinates total patient care, serving as the primary nurse for designated patients and evaluating the effectiveness of care.
- Current CRNP license in the state of Pennsylvania.
- 2-3 years' experience in patient care required.
- Excellent clinical skills, including CPR certification.
- Solid computer skills, including Word.
- Excellent organizational skills with attention to detail.
- Outstanding communication skills (written and verbal).
