Job Title: Physical Therapy Assistant

Overview:

UF Health is seeking a skilled Physical Therapy Assistant to provide high-quality patient care in a home health setting. As a Physical Therapy Assistant, you will work under the supervision of a Physical Therapist to deliver evidence-based therapy interventions and promote patient independence.

Responsibilities:
  • Contribute to the evaluation process by implementing delegated assessments and providing verbal and written reports of observations and client capacities to the supervising therapist.
  • Provide input for intervention plan, based on client needs and priorities, and be knowledgeable about the client's therapy goals.
  • Observe, record, and report the client's reaction to treatment and any changes in the client's condition to the supervising Physical Therapist, physician, Director of Nursing, Administrator, and/or Clinical Supervisor as needed and in accordance with agency policies.
  • Educate and document in the patient record instructions given to patient, caregiver, and/or family member or other healthcare personnel.
  • Submit accurate documentation in accordance with agency policies.
  • Participate in meetings and on committees, as well as educational opportunities to continually improve personal performance and overall home care excellence.
  • Participate in the agency's quality assessment and performance improvement program.
  • Under the guidance of the supervising therapist, treat to relieve pain, develop and restore function, and maintain maximum performance using appropriate treatments, modalities, and procedures as stated in the plan of care.
  • Promote coordination of care through interdisciplinary communication in accordance with agency policies.
  • Participate in instructing the client, family, caregiver, and/or other healthcare team personnel in the exercise program developed by the Occupational Therapist for strengthening and controlling the client's upper extremities.
  • Uphold the highest ethical standards in relation to interaction and work with patients, physicians, and other community contacts.
  • Meet the productivity standards that have been established by the agency.
Qualifications:
  • Graduate of an APTA accredited Physical Therapy program required.
  • 2 or more years experience in home health preferred but not required.
Additional Requirements:
  • Level II background check required for position.
  • Must have a good driving record.
  • Valid driver's license and proof of auto insurance required for Motor Vehicles Record (MVR) background check.

Post-Employment: BLS, provided by American Heart Association (AHA), required within 30 days of hire.
