Key Responsibilities

* Develop and implement individualized plans of care that address the unique needs of each patient
* Conduct thorough assessments to identify patient goals and objectives
* Collaborate with healthcare teams to ensure seamless coordination of care
* Administer medications and treatments as prescribed, and monitor patient responses
* Educate patients and their families on disease management, self-care, and prevention strategies

Requirements

* Current Registered Nurse (RN) license for the state in which you practice
* Current health certificate
* Current PPD or Chest X-Ray
* Current BLS card
* One year prior RN experience preferred
