Care Coordinator for Long Term Care

We are seeking a skilled MDS Coordinator to provide care to assigned residents in a long term care facility.
Job Type: Registry, Full-time
Work Schedule: 8:00am - 4:30pm
Estimated Salary: $69.00 per hour
About the Role:- Provide high-quality care to assigned residents in a caring, safe and efficient manner.
- Perform tasks according to location standards, procedures and individualized resident care plans.
Responsibilities:
- Complete nursing portion of Minimum Data Set (MDS) with accuracy and efficiency.
- Maintain residents' self-esteem, privacy and confidentiality of personal information.
- Assist in maintaining safe, pleasant, orderly resident living areas.
- Review and sign MDS forms for accuracy and validity.
- Provide quality check prior to MDS transmission.
- Query other members for accuracy of MDS.
- Observe and report residents' conditions.
- Communicate effectively with physicians using center/campus systems.
- DIRECTS AND ASSISTS WITH CO-WORKER, RESIDENT, FAMILY/SIGNIFICANT OTHER EDUCATION.
- Utilize Electronic Medical Record (EMR) accurately.
Requirements:
- RN license is required.
