Family Medicine and Internal Medicine Specialist Needed

We are seeking a highly skilled Family Medicine and Internal Medicine physician to join our team in Middletown, NY. As a member of our Optum Care Delivery team, you will be an integral part of our vision to make healthcare better for everyone.
As a Physician-Led, Patient-Centered Team-Based Care Environment, you will have the opportunity to practice at the top of your license, focusing on quality rather than quantity while delivering exceptional care to patients. You will also have moderate scheduling templates to promote work-life balance, allowing you to partner with other medical professionals while practicing with autonomy.
Primary Responsibilities:
- Direct and Manage a Complete Continuum of High-Quality Care within the Care Team
- Treat Chronic Diseases and Disorders
- Review Patient Files/Records, Including Allergies, Problems, Medications, and Immunization Status
- Elicit and Record Information About Patients' Medical Histories
