CentraCare Health System is seeking a full-time Family Medicine Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team-based care model in Long Prairie, Minnesota.
Responsibilities:- Develop and manage your own patient panel in the clinic, providing independent care for patients of all ages.
- Diagnose and treat short-term illnesses, provide preventive care, and manage chronic conditions.
- Support the walk-in clinic and collaborate with the physician group and facility as needed.
- Completion of a formal nurse practitioner or physician assistant program approved by a national accrediting agency.
- Certification as a Nurse Practitioner or a Physician Assistant.
- Current or pending licensure in the State of Minnesota, licensed to see pediatrics through geriatrics.
- BSL Certification; ACLS and CALS.
- Relevant work experience in Family Medicine, Hospital Medicine, Internal Medicine, or Walk-In Clinic/Urgent Care.
