About the Position

As a Home Health Risk Assessment Specialist with Matrix Medical Network, you will conduct in-depth assessments of patients' current health, medical history, medication adherence, social environment, and other risks during visits that can last up to one hour. This provides unparalleled insight into a patient's overall health and well-being, enabling us to improve quality of care and potentially close multiple care gaps with a single visit.

Responsibilities
  • Conduct comprehensive adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.
  • Work closely with Primary Care Physicians (PCPs) on patient education and provide follow-up support.
  • Provide services in various settings, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree or equivalent experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the AANP, ANCC, or AACN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Comfortable and flexible with frequent change.
  • A valid state driver's license and proof of adequate automobile insurance coverage for the state of residence.
Compensation

$90.00 per completed assessment. Flexible scheduling options available.
